Property Information Form

This is the form to be filled by an individual or an organization duly represented in which the entity is acting in the capacity of an owner of land, lots or any type of building. Land use must be specified may be for personal use or commercial. It must be filled by the property owner who has enjoyed ownership for more than three years.
